Where is your office located?
Conveniently located at 1823 NW Kings Blvd in Corvallis, OR, you can find our office just south of the Kings Circle Shopping Center between NW Circle Blvd and NW Garfield Ave.
What types of insurance do you accept?
At Evergreen Dental, we accept most major dental insurance plans, and maintain preferred provider status with Aetna, Blue Cross Blue Shield, MODA, Cigna, United Healthcare, Pacific Source, MetLife, and Advantage. We also happily accept cash, credit and debit as payment options.
What types of financing do you offer?

How often should I schedule an appointment?
Dr. Travelstead and the American Dental Association recommend that patients schedule a dental exam and cleaning once every six months. Regular exams provide Dr. Travelstead with the opportunity to spot the early signs of tooth decay and gum disease while these types of problems are still easily treatable. Frequent cleanings provide our team of gentle dental hygienists with the chance to remove plaque deposits from the surface of your teeth and along the gum line before the buildup of bacteria can contribute to the development of gum disease.
